Your registration will not be fully complete until you follow ALL 3 steps below.
Forms need to be filled out by all YOUTH and ADULTS attending Trek.
1. Fill out the online REGISTRATION (google form)
Please be prepared with emergency contacts, medical info, medications, physician and insurance info before you begin.
After submitting registration step 1 you will have to return to the registration page to complete step 2 (submitting will not automatically take you to the next step)
All YOUTH attending Trek, click here to register
All ADULTS attending Trek, click here to register
2. Sign the Church Permission & Medical Release Form, the Trek Site Permission Form and the Stake Hold Harmless Form.
When you click the link below to open up the forms, you will be prompted to enter your name and email. Please use the parent name and parent email to open the forms. A copy of the form will be sent to this email. (see image below)
If a field is not applicable to you, put N/A
When you are finished filling in all the required fields, make sure you click the green "Sign and Complete" in the upper right corner of the page. (see image below)
You will ALSO need to click the green "Accept & Send." (see image below)
Finally, you should see a message that says "You've successfully signed the document!" (see image below)
